Conference is aimed at: LTHT ward based palliative & end of life care link nurses, physiotherapists, occupational therapists, speech & language therapists, dietitians, radiographers, pharmacists, medical social workers & chaplaincy

The objectives for the day are to:

Inspire delegates to act as clinical champions, raising the profile of end of life care in their working environment.

Enhance knowledge regarding the latest national and local end of life care initiatives.

Recognise and make the most of opportunities to discuss choices with patients with life limiting illnesses, as end of life approaches

Gain understanding of how to implement individualised care planning including shared decision making in practice

Improve confidence in addressing the care & comfort of patients at the end of life and their families
